Pick your clinic, add your name and email, and copy the signature into Outlook. The logo, colour, address, phone and links are filled in from the clinic's record — change any of them if something has moved on.

Outlook on Windows: File → Options → Mail → Signatures → New, name it, click into the editing box, then press Ctrl+V.
Outlook on Mac: Outlook menu → Settings → Signatures → +, name it, click into the editing box, then press Cmd+V.
Outlook on the web: Settings (gear) → Mail → Compose and reply → paste into the signature box, then Save.
Paste as-is. Don't retype or reformat the text afterwards — that strips the layout. If the logo doesn't appear, your mail app has blocked images for that message; it will show for the people you email.
